Please also note that during your therapy program I identify you with your email address. I don’t use (or ask for) your full name, date of birth or other demographic data. I ask for your first name so I can greet you personally. Therefore, whenever you submit something online I will ask for your email address so that I know it’s you. When you email me information or forms I will automatically know it’s you from your email address - but if you have more than one email address, please remember which one you are using as your identifier for your work/sessions with me.
